Provider Bidding w/ Expired License

Provider bidding has an expired license

  1. Copy the license number from the provider profile in CMS

  2. Find the State or Commonwealth’s licensing board website by clicking here: If you do not receive a result, Google “State name” + Verify Massage License” to find the correct link.

  3. Search license number

    1. Alert: Confirm the number matches the provider’s historical data in CMS.

  4. Confirm the name on the professional license matches the provider’s name in the CMS.

    1. If the name does not match exactly, reassign the ticket to the Credentialing team and queue in Kustomer with a note saying what the discrepancy is.

  5. Confirm the license status is active, and the expiration date is in the future

  6. Confirm there are no stipulations or enforcement actions against the provider’s license. Some websites have stipulations or enforcement actions listed on a separate page from the license lookup page. Other websites require you to click “more details” next to the provider’s name. If there are stipulations listed, flag the name in the Trust and Safety Slack channel

  7. If the License is active, the names match, and there are no disciplinary actions against the provider, update the license expiration date on the provider page in CMS.

  8. Create a Kustomer note or reassign the ticket to the Typeform team and queue and please add the “CMS updated” tag. Keep the ticket Open.
