Provider Bidding w/ Expired License

Provider bidding has an expired license

1. Navigate to https://www.fsmtb.org/license-lookup/, and verify the provider’s state license appears as “Active” on the state website. If the license is valid

2. Copy the License Number. Delete ALL expired licenses for that state. Update the license in CMS by navigating to the provider’s profile, scrolling to the license section, deleting ALL expired licenses for that state before adding a new license using the plus sign icon. 

   1. Make sure to add the requested information into CMS from the state site including license number and expiration date. 

   2. Speciality = Massage Therapist

   3. State = Whichever state website 

   4. License Number = “Copy from expired license before deleting OR from state website”

   5. Year = 0 

3. Confirm the provider on the appointment.
