Login to ZCC and navigate to the "Tasks" tab
Filter for:
- Task Type: Copy Appointment to HSRM
- Milestone Appointment: Final
Sort list by VA Facility Name
Once on the filtered grid, scroll to the right and for each row, you'll begin by clicking the green “Copy Appointment to HSRM” button
- Copy Referral Number
Login to HSRM
- Click into your desired NPI list, hit the 3 horiontal bars, and select "Find referral"
  - You may need to check multiple referral lists in HSRM to find the referral. You can narrow this down by VAMC and eventually you'll have an idea of which providers will only be in the in-office list.
- Once you find your patient's referral in HSRM, click into it
- On the right side of the screen, click “Add/View Documents”
- Go Back to ZCC
  - Click “Choose specific files to include”
  - Click Soap Notes and Evaluation
    - Ensure both are checked and the dates match.
    - ENSURE THERE ARE 12 SOAP NOTES
    - You can include all evaluations as well.
  - Click Create PDF.
  - Click Edit Pencil Icon on PDF Ready Popup
  - Rename File to "First initial last name SOAP notes 2-12 and evaluations DATE
    - ie. D Smith SOAP notes 2-12 + Evals 2/12/24
- Go back to HSRM
  - Add New Documents in bottom right
  - In the “Add or drop new documents box” upload this new PDF.
  - Set Description to “SOAP notes 2-12 + Evaluations”
  - Change Document type to “Medical Documents”
  - Click Update
- Go Back to Referral Details
  - Scroll to the Note Section at the bottom of the page
    - In the comments, add “MM/DD-Uploaded all SOAP notes / evaluations"
- Go back to ZCC and click "Finish".
