PCC Is Vaccine Medically Appropriate

Use Case: 

• Patient: is vaccine medically appropriate due to existing condition

Response: 

• If nurse notifies you from on-site:

  • Hello, NAME, thank you for letting us know. Please provide a brief description of the medical conditions that you or the patients are concerned about and wait for a response from Dr. Peters before vaccinating the patient.

• If patient off-site:

  • Thank you for making us aware. We advise that you check with your Primary Healthcare Physician for approval before scheduling your Vaccination appointment. (Clinical team/Dr Peters to talk to physician- PCC to coordinate)

Action: 

• If On-Site: Escalate to T + S in Slack. Have nurse send explanation of patients medical condition and tag Dr. Peters. Text Dr. Peters the nurses name, but no PHI. Advise nurse to NOT vaccinate until after Dr. Peters approval

• If Off-Site: Advise patient to reach out to Primary Healthcare Physician
