PCC Cancellation of Vaccine Appointment

Use Case:

  • Cancelation of vaccine appt by provider or patient

Response: 

  • Before appt in Voicemail: Update records in Salesforce

  • During appt (nurse often notifies us): update records in SalesForce, notify Homebound-cancellations-standby.

Action: 

  • Pt: Call the patient to inform that we received their voicemail, ask why they are canceling. Will they stay on our list to reschedule, or remove?

  • Notify nurse we have received their message about cancelation, ask reason for cancelation, find a standby

  • Provider: Rebook and update patient on any revised ETA. Alert T&S to habitual unprofessional cancellations or late-arrivals
