Use Case: Patient's assigned provider is no longer on the network, does not remember treating the patient, refuses to submit RFS, or otherwise cannot/will not submit an RFS for the patient.

Hello NAME,

Please answer the following questions so that we may submit an RFS to your VA facility.

1. Please state what your chief complaints were at the start of the sessions (body parts and pain scales)

2. What were your functional limitations (walking, sleeping, standing, running, etc)?

3. What progress has been made to date with regard to the pain and functional limitations as a result of massage therapy?

4. What are your specific goals for future sessions?

5. What functional goals do you plan to improve upon?

Action :

• Notate ZCC that questions were sent and/or answered.

• Assign replies to the Clinical Kustomer queue.
