Handling "Name Not Matching" Errors

These errors occur when a patient has two different names listed in the system, preventing the system from identifying the correct name associated with the referral.

Step-by-Step Instructions

  1. Access the Name Discrepancy Sheet
    Open the following spreadsheet to view existing name discrepancies:

  2. Locate the Referral in ZCC

    • Copy the referral number from the spreadsheet.

    • Paste it into the ZCC system to pull up the patient record.

  3. Access Patient Information

    • Once the patient record appears, click on their information tab.

    • On the right-hand side, scroll down to the "Attached Files" section.

  4. Review the Referral Form

    • Click on the referral form listed in the attached files.

    • When the document loads, the patient’s legal name will appear at the top.

  5. Update the Spreadsheet

    • Add both names to the sheet:

      • The name as it appears in ZCC

      • The legal name shown on the referral form

  • Some sections of the referral form may contain long notes.

  • Ignore these; focus only on the fields where a name is listed.
