When logging into HSRM you will need to log in with ID.me and enter the code sent to your phone.
The next page you are brought to is split up into six different sections. These sections are:

1. In-home referrals
2. Southeast Region Offices
3. Northeast Region Offices
4. Midwest Region Offices
5. West Region Offices
6. Behavioral Health Referrals
There are two different ways you can search for a referral:

Referral Number: Searching by referral number will allow you to locate one specific referral. First select the region from the main page, then click the three lines in the top left corner and select “Find Referrals”, paste the referral number in the first section labeled “referral number” and click “Find” in the bottom right corner.

By Veteran: Searching by Patient will allow you to pull up all referrals assigned to us in HSRM for a specific veteran. First select the region from the main page, then click the three lines in the top left corner and select “Find Referral by Patient”, you will then need the veteran’s first name, last name, and date of birth which can be located in the Referral tab in ZCC, once these have been entered into the correct sections click “Find” in the bottom right corner.

To get back to the page with the different regions, you will click on your name in the top right corner, then select “Referrals by NPI”
