Blasting and Reblasting Appointments

In order to sent a request out to the network the appointment needs to be "blasted".

Blast lists typically generate automatically, but if we clone an appointment we need to create the blast list.

When a blast list is created or an appointment is reblasted, the providers will be notified in order of their tier according to the blasting logic. If we want to send the notification immediately, we need to click the blue flame next to the group.
